Please download, print, and fill out the necessary forms to bring with you to your pet's appointment.
This form needs to be completed every time you board a cat with us.
Please fill this out just the
first time you are boarding a cat with us. We will keep it on file.
Please fill out this form if you are a new client at EBAH or if your address
and information has changed and we need to update your record.
Please fill out this form if your pet is staying the day with us. No other exams are needed,
no bath is needed, etc.
This form needs to be completed every time you board a dog with us.
Please fill this out just the
first time you are boarding a dog with us. We will keep it on file.
Please fill out this form if you are out of town and have authorized someone
else to bring your pet into EBAH for medical attention.
Forms For Hospital Admissions:
Please download, print, and fill out the necessary forms to bring with you to your pet's appointment.
Please fill out this form if your cat is scheduled for an annual physical exam with a doctor.
Please fill out this form if you are a new client at EBAH or if your address
and information has changed and we need to update your record.
Please fill out this form if your pet is being admitted into our
hospital for a "problem" exam not scheduled at a particular time of the day with a doctor.
This may include shaking of head, limping, skin conditions, vomiting, diarrhea, etc. or
other scheduled appointment.
Please fill our this form if your pet is scheduled for a dental cleaning.
Please fill out this form if your dog is scheduled for an annual physical exam with a doctor.
Please fill out this form if your dog is scheduled for a skin/allergy exam.
Please fill out this form if you are admitting your pet
into our hospital for lab testing (urine or bloodwork), bath, x-rays, or other scheduled appointment..
